Bellevue Healthcare Center operates on behalf of Region Skåne. The healthcare center has approximately 4,000 listed patients and employs doctors, nurses, nursing assistants, a psychologist, a physiotherapist, and others.

Description of duties:

We are looking for a curator who will also work as a rehabilitation coordinator at Bellevue Healthcare Center and the Health Center Malmö. In this role, you have overall responsibility for rehabilitation coordination for patients.

The position involves working with primary care assignments, assessing and treating patients with mild to moderate mental health issues, including iCBT treatments, as well as coordinating and monitoring rehabilitation efforts for patients who need support in their recovery after illness or injury.

As a curator, you will meet patients with common conditions such as depression, stress-related health issues, anxiety disorders, and sleep difficulties. You conduct assessments, initiate treatments, and handle initial contacts with patients. When needed, you coordinate further efforts within the care team. Together with the clinic's doctors, you participate in joint rounds where patients are discussed.

In the role of rehabilitation coordinator, you have overall responsibility for planning, coordinating, and monitoring rehabilitation efforts for patients with varying needs. The work includes, among other things, establishing rehabilitation plans, collaborating with physiotherapists and other care staff, and supporting patients in planning their return to work or other activities. You serve as an important link in the care chain and ensure that patients receive coherent support and continuous follow-up throughout the rehabilitation process.

To succeed in the role, you need:

• A degree in social work and training as a rehabilitation coordinator.
• You must have training to be able to conduct iCBT treatment.
• You will work across the entire organization and coordinate rehabilitation efforts for patients listed at all four of our healthcare centers.
